SSM is looking for an experienced LPN with Hospice/Liaison or Case Management experience that has a dynamic personality with a ton of energy and passion for Hospice and Home base care. We are looking for a nurse with Hospice, Case Management, or Home Health background!!!
The selected individual will be responsible for;
• Meeting with patients/families in person to discuss continuum of care services.
• Meeting/communicating with referral sources (case management, social work, clinics, providers, SNF/ALF facilities etc).
• Care coordination between continuum of care departments, such as branch departments, business development etc.
• Eligibility/Chart Review to determine appropriateness for continuum of care services
***Schedule is roughly 8-5 with on call rotation roughly every 10 - 12 weeks
***$5,000 Sign on bonus eligible for qualified candidates with a 24 month commitment! We also offer a full benefit package that starts on the first day of the month after you start! Night and Weekend differentials plus.70/mile!
Job Summary:
Provides care to patients under the direction of a registered nurse or physician, functioning within the scope of license.Job Responsibilities and Requirements:
PRIMARY RESPONSIBILITIES
- Delivers care to assigned clients and performs direct care in accordance with an established plan of care.
- Delivers safe care at all times that is in line with physician orders, home health policy and established nursing care standards.
- Maintains communication regarding patient progress with physician, supervisor, staff and family members as appropriate to the situation. Reports patient-related observations to supervising RN on a daily basis.
- Administers medications and observes patients for adverse reactions to medications or treatments.
- Provides documentation that follows the established treatment plan, supports coordination of patient care, meets regulatory requirements and ensures reimbursement.
- Works in a constant state of alertness and safe manner.
- May perform point of care testing according to policies and procedures.
- Performs other duties as assigned.
EDUCATION
- Graduate of an accredited school of nursing or educational equivalency for licensing
EXPERIENCE
- One year licensed practical nurse experience
PHYSICAL REQUIREMENTS
- Constant use of speech to share information through oral communication.
- Constant standing and walking.
- Frequent lifting/carrying and pushing/pulling objects weighing 0-25 lbs.
- Frequent sitting, reaching and keyboard use/data entry.
- Frequent use of vision and depth perception for distances near (20 inches or less) and far (20 feet or more) and to identify and distinguish colors.
- Frequent use of smell to detect/recognize odors.
- Frequent use of hearing to receive oral communication, distinguish body sounds and/or hear alarms, malfunctioning machinery, etc.
- Occasional lifting/carrying and pushing/pulling objects weighing 25-50 lbs.
- Occasional lifting/moving of patients.
