Job Description Summary:

 

The Care Connection Project Manager manages project coordination of school-based health initiatives, optimizing outcomes by maintaining internal and external partnerships, integrating a quality improvement structure, and implementing strategic community engagement by directing a team of liaisons to achieve partnership satisfaction and effectiveness. Assists the Director of School Health Services with programmatic evaluation.

 

Job Description:

 

Essential Functions:

• Manages Community Wellness school-based projects, facilitating cross-functional projects, building relationships, and setting goals to achieve desired outcomes.
• Oversees the development of data collection tools, data analysis, and data management to support continuous quality improvement, reporting, and research needs. This includes coordinating cross-systems data analysis with school and community partners.
• Supervises School Health Liaisons, setting performance goals, establishing priorities, and providing expectations of job function in alignment with organization’s mission, vision, and values.
• Recruits, trains, evaluates, develops, and as necessary, disciplines and/or terminates subordinate staff. Guides subordinate staff in the utilization of data to promote a proactive culture and to develop a team attitude that is open to change.
• Represents organization in city, state and national committees directing school health integration as assigned by Director.
• Monitors, evaluates and justifies engagement budget expenditures. Judges appropriateness based on effects of program outcomes, community and partnership needs.
• Promotes teamwork by building a team environment and fostering cross-functional collaboration. Communicates effectively with internal multi-disciplinary teams, external community leaders, patients & their families to resolve problems and provide quality service.

Education Requirement:

Bachelor’s Degree is required, Masters preferred. Work requires a level of knowledge equivalent to that obtained by completing a master’s degree program in public health, health services management & policy, healthcare administration or closely related field.

Licensure Requirement:

Valid Ohio driver’s license and proof of auto insurance as required by hospital policy. Must pass motor vehicle background inspection, insurance eligibility, driving qualifications, and training set forth by Nationwide Children’s Hospital and maintain qualification of insurance guidelines.

• Flexible and sensitive to the needs of the patients and families we serve.

Experience:

• Three years of experience in program management and leadership desirable.
• Experience in applying the IHI’s Model of Improvement in community-based platforms preferred.
• Supervisory experience required to plan, assign, and evaluate the work of subordinate staff.
