Registered Nurse (RN) - Patient Flow Coordinator - PRN Days

Job Summary

To support current initiatives to maximize our capacity. This will allow for more timely transitions of care from our Emergency Department and procedural areas. The Patient Flow Navigator will work closely with the Emergency Department, inpatient units, Patient Placement, Administrators on Duty, and Environmental Services. The Patient Flow Navigator will hardwire standardized processes and monitor transitions of care.

• Standardizes processes to ensure patients are admitted to the appropriate unit with the appropriate orders.
• Hardwires standardized processes for inpatient transfers and patient discharges. 
• Evaluates bed turnaround times and develops action items to address opportunities.
• Supports efforts to improve patient throughput and ensure safe, timely care transitions for our patients.
• Strengthens utilization of current systems to support patient throughput.
• Collaborates with the Emergency Department, inpatient units, Administrators on Duty, and Environmental Services to improve care transitions.
