Your Specific Responsibilities:

This is a professional nursing position specializing in providing comprehensive nursing care to pre and post-natal women, as well as offering case management for infants and management for families of these women.  This position is a team leader for the Healthy Start program and is under direct supervision of the Registered Nurse Supervisor. This position covers the Healthy Start program for the Dixie/Gilchrist CHDs.  Case management of Healthy Start clients:  e.g. arranging transportation, helping to acquire housing, Medicaid and food stamp applications.  Coordinates, facilitates and provides Healthy Start care coordination that meets the minimum standard requirements and assures risk appropriate care under the new Medicaid Waiver Program.  Responsible for completing PEPW application for eligible clients. Review Healthy Start screening instruments, statistical data and reports.  Reviews and reconciles monthly Healthy Start reports, data input, and QA/QI reports. Teach educational classes such as Childbirth Education, Parenting, Breastfeeding, Smoking Cessation Support groups and classes in response to client requests and needs. Perform home visits to Healthy Start clients and provide individualized education and developmental screenings while there. Assesses individual clients’ needs and develops Individualized Plans of Care and follow-up care.  Refers to appropriate resources, when indicated.  Responsible for implementing QI review per Healthy Start guidelines. Performs other duties as requested by supervisor.

Required Knowledge, Skills, and Abilities:

Up-to-date knowledge of maternal/infant nursing care, childbirth education and breastfeeding; excellent verbal and written communication skills; basic computer skills; knowledge of community resources; ability to effectively interact with the community.

Qualifications:

Minimum –Clear and Active Florida Registered Nurse License, valid driver’s license, reliable transportation and willing to travel from site to site, basic computer skills, good written and verbal skills.                           

Preferred – Experience working in HMS in a County Health Department, experience working with pregnant women and their families, experience working with children from birth to 3 years old and their families, experience working in a position that requires client home visits, case management experience.
