Cornerstone Care, a Federally Qualified Health Center (FQHC), with locations throughout Southwestern Pa., is seeking a Medical Records Clerk to join our team.

POSITION SUMMARY: Responsible for maintaining, organizing, and processing patient medical records in accordance with regulatory standards and organizational policies. This role ensures accurate and timely documentation, supports clinical and administrative staff, and safeguards the confidentiality of patient information.

JOB DUTIES AND RESPONSIBILITIES:

 

  • Receives, organizes, and files patient medical records, both electronic and paper, ensuring accuracy and completeness. 
  • Retrieves medical records for staff, providers, and authorized individuals in a timely manner. 
  • Reviews records for compliance with documentation standards, notifying providers of missing or incomplete information. 
  • Maintains confidentiality and security of patient information in accordance with HIPAA regulations. 
  • Enters and updates patient demographic and clinical information into electronic health record systems. 
  • Processes requests for medical records, including release of information requests, ensuring proper authorization. 
  • Assists in audits, quality assurance, and compliance activities related to medical records management. 
  • Responds to internal and external inquiries regarding medical records, maintaining professionalism and discretion. 
  • Assists in transitioning paper records to electronic formats when necessary.
