About the role

Summary

The Division of Public Health Pediatrics is seeking a Manager-Community Health to lead the upREACH initiative, a newly expanded community-based program funded by a four-year, nine million dollar grant. This role will oversee program implementation, supervise a team of community health workers, and ensure alignment with the Division’s strategic goals. The manager will collaborate closely with nurses and the nurse manager to support participants with medical needs and strengthen care coordination. Ideal candidates will demonstrate strong leadership, program management experience, and a commitment to advancing health equity in underserved communities.

Job Duties

  • Provides leadership and management to the upREACH team, including supervising community health workers who deliver the intervention, creating and implementing a recruitment strategy for the program, and participating in strategic planning for upREACH and upSTART programs.
  • Leads program quality improvement activities, including identification of areas of improvement, developing plan with staff and upSTART executive leadership, tracking progress.
  • Assists with the development of program database and tracking tools; ensures all program data are entered in a timely manner.
  • Reports on program progress and challenges during upSTART leadership meetings and provides needed information for funding reports.
  • Co-leads the Division Welcoming Committee - creates and maintains onboarding documentation and supports employee wellness activities. Participates in Division Leadership meetings. 
  • Assists with data analyses and preparation of mansuscripts and creates program evaluation reports.
  • Performs other duties as assigned.

Minimum Qualifications

  • Bachelor’s degree in a related field.
  • Four years of relevant experience.
