POSITION SUMMARY
The Community Wellness Department (CWD) is a multi-site program operating in San Francisco and Oakland, that provides culturally sensitive prevention, early intervention, and supportive social service programming. CWD community programs include social service programming for youth and adults, cultural programming, health education, system navigation/ coordination services, and HIV/AIDS prevention and care. Services are rooted in a holistic provision model that integrates prevention, community engagement, cultural competence and provides linkages to other treatment services.
The Community Wellness Department is seeking a Senior Program Manager to collaborate with the CWD Director to manage day-to-day operations of the department areas that include monitoring grant performance, assist with budget management, lead program planning, guide QMIP and other Quality Improvement initiatives, operationalize reporting and evaluation. The incumbent will lead, maintain, and expand CWD’s Comprehensive
Perinatal Services Program (CPSP).
The Community Wellness Department provides multi-site services in 2 Bay Area Counties at 2 locations (1 in SF and 1 in Oakland) to urban American Indians, Alaska Natives, other Indigenous people, and diverse members of the surrounding communities.
The Native American Health Center is an accredited institution and adheres to the standards of excellence set forth by the Accreditation Association of Ambulatory Health Care (AAAHC) and the Commission of Dental Accreditation (CODA).
DUTIES AND RESPONSIBILITIES
1. Co-Manage CWD’s day-to-day operations, project performance management, hiring, human resources management and ensuring effective spending.
2. Function as an integral member of the department’s leadership team, working closely with the Director of Community Wellness Department, Senior Program Managers, Director of Behavioral Health, and Director of Research & Evaluation.
3. Support departmental strategic planning initiatives and engage in department development, performance management, and team building.
4. Implement and sustain the CPSP Perinatal expansion project and develop workflows for cross-functional division, CWD WIC, and Medical
5. Develop and sustain the women's Children and Family service division for CWD, a new revenue producing multi-departmental service line
6. Develop, implement, and maintain operation processes, procedures, and collaboration with the State, Federal, and County Home Visiting Programs.
7. Consult with leadership on the program modifications and recommendations for State, Federal, and County contracts.
8. Convene and facilitate joint planning to design and implement State, Federal, and County Home Visiting Programs.
9. Manage and Lead the daily functions of four direct service programs, Tribal Home Visiting, Fatherhood, Alameda County Case Management, and CPSP Perinatal expansion project.
10. Monitor project implementation and program rollout tracking, communications, and reporting for internal and external stakeholders.
11. Conducts audits of database and relevant processes in order to identify problems and perform necessary corrections.
12. Manage and Lead the daily functions of four direct service programs, Tribal Home Visiting, Fatherhood, Alameda County Case Management, and CPSP Perinatal expansion project.
13. Resolve staffing issues and facilitate conflict resolution for staff under direct supervision. Hire, discipline, evaluate and terminate staff in coordination with the Human Resources department.
14. Support the execution and implementation of initiatives across the department and multiples sites, coordinating with site managers, and analyze data to ensure compliance with accrediting bodies and funding sources.
15. Ensures patient and program data is accurately and completely recorded in the agency’s electronic record health system according to contractual timelines and requirements.
16. Provide leadership on program development and expansion of program activities to support grant writing. Duties include writing and collecting supporting documents and directing grant communications with funders.
17. Coordinate activities across departments, sites and with collaborating agencies to ensure that program areas are meeting goals and objectives.
