AdvisaCare is seeking a Home Health Field RN and LPN to join our team in Grand Rapids. Must hold an active Michigan state Nursing License. The right candidate should be self-motivated, driven, out-going and efficient with their charting and review of documents.

Monday-Friday 8:00a-5:00p mostly in office, but the need for assistance with occasional staffing cases will be required if needed. Supervisory visits with clients and field staff will also be done by the Clinical Supervisor routinely.

Job Description for Home Health Nurse:

• Case Opening Assessments
• Care Plans
• Medication set up and reconciliation
• Wound care visits
• Supervisory and nurse visits
• Timely paperwork completion that meets DOH Compliance Standards
• Must be willing to travel within our main coverage area
• Positive team player who will also advocate for our clients and their needs

Requirements

• Current MI State Nursing License, TB test, CPR
• Clear Criminal Background Check and Drug Screen
• Detail oriented, ability to multi-task, organized with good time management skills
• Reliable transportation, MI Driver's License & Auto Insurance
• Experience with Wounds, Assessments, Medication set-up & Management
• Current and reliable cell phone and be easily reachable
