Nurse Care Manager

The Nurse Care Manager is responsible for the care management of patients with acute and chronic medical conditions and the assessment of care needs of vulnerable populations for various clients.
Hours: Monday through Friday from 8:00 am to 5:00 pm.

Essential Functions

• Perform assessment and evaluation of potential care management patients per client contractual obligations and/or state requirements.
• Collaborate with health care providers and ancillary service providers to validate the appropriate treatment plan and identify educational opportunities.
• Assessment/education may include face-to-face interaction for specific contracts.
• Provide ongoing education.
• Communicate with all health care providers to ensure appropriateness of care.
• May recommend and make adjustments in care if needed.
• Evaluate treatment adherence and reevaluate the patient’s commitment to make the necessary changes.
• Documentation per contractual requirements related to individual member status, and/or medical necessity review, and negotiation.

Requirements

• Current and active RN in Mississippi
• Minimum 1 year experience in human services and/or 3 years clinical practice experience.  
• Experience in human services is strongly preferred, such as a Qualified Intellectual Disability Professional (QIDP)
• Experience as a Case Manager
