Overview
The social worker works under the supervision of the Administrator to assist each patient/resident in reaching their maximum level of social and emotional functioning by promoting, interpreting and advocating their needs to family members and LLHARS interdisciplinary team. The Social worker is in charge of discharge planning, care planning, discharge notifications, PASRR forms, Medicaid applications, psychoactive monitoring, behavior monitoring, social and psychosocial needs of the residents. The social worker is a resource for the resident and resident family members related to care and services. The social worker will spend time meeting with residents and resident family members. The social worker is an active participant in the interdisciplinary team in coordinating care and services for each resident.

What you will do
- Documentation is completed accurately, including all required components in accordance with Medicare/Medicaid/State survey regulations and is placed on the medical record within the appropriate time frame.
- Provides exposure to, and an understanding of those services/programs in LLHARS that can enhance the patient's quality of life and independence as they transition back to their community.
- MDS assessments are completed as assigned within the appropriate time frame and without significant corrections.
- Resident and family concerns/complaints are addressed within 24 hours, 95% to 100% of the time.
- Responsible for all discharge planning, care planning, discharge notifications, PASRR forms, Medicaid applications, psychoactive monitoring, behavior monitoring, and assessment of the social and psychosocial needs of the patients.
Qualifications
Education
- Bachelors Degree, Social Work or Related Field (Required)
Experience
- 1 year Prior work experience in a geriatric or healthcare setting (Preferred)
Licenses and Certifications
- Certified Auto External Defibrillator (AED) - American Heart Association/American Red Cross/American Safety and Health Institute (AHA/ARC) within 30 Days(Required)
- Licensed Clinical Social Worker (LCSW) - Virginia Department of Health Professions (VDHP) (Preferred)
