Sr. Associate, Network Integration
The Sr. Associate, Network Integration will be responsible for network integration activities including identifying, outreaching, and building relationships, and long-term relationship management on behalf of Strive Care Partners. This person will onboard and collaborate with both Nephrologist partners as well as community providers including specialty provider practices and facilities that contribute clinical value to our care model. The Sr. Associate will support the development and execution of the Network Integration centralized playbook for how to effectively work with these providers across SCP (Strive Care Partners) SC365 and CKCC markets in coordination with the Market Operations leads. Sr. Associate, Network Integration will play a crucial role in driving seamless collaboration and optimizing Nephrology Provider and Specialty Provider engagement initiatives that contribute to the overall growth and success of SCP.
Essential Functions
Nephrologist Provider Relations
- Work with internal Strive providers to collaborate and build relationships with Nephrologist partners to establish and active referral pipelines
- Facilitate relationship building among SCP providers and Nephrologists to promote collaboration, knowledge sharing, and best practices of SCP
- Develop and implement tracking mechanisms to monitor the success and impact of provider engagement initiatives in each market
- Analyze data, generate reports, and identify trends and insights and use findings to propose actionable recommendations and refine strategies for continuous improvements
- Work with market operations to develop standardized reporting and communication to key stakeholders in market
- Support initiatives regarding provider facing roadshow sessions to help educate providers on SCP care model
- Work closely with internal teams, including sales, marketing, operations, and technology, to align strategies and initiatives
- Collaborate on joint projects and initiatives to maximize integration efforts and enhance overall organizational performance
- Meet and exceed targets for physician engagement activities
Specialty Provider Relations
- Develop and execute strategies to effectively engage healthcare providers including SNFs, hospitals, rehab facilities, cardiologists, endocrinologists, and other types of community providers
