About the role
The Senior Director of Clinical Care Coordination is a strategic clinical leader responsible for overseeing care coordination, clinical escalations, case management, and client placement across the CCS network. This role ensures that high-quality, person-centered care is delivered consistently and effectively, with a strong emphasis on clinical outcomes, provider collaboration, and operational excellence.
The Senior Director leads multiple teams, including the Clinical Case Management Team, and is accountable for ensuring that client families progress through their care paths with appropriate consultation, oversight, and support. This role drives clinical strategy, optimizes care delivery systems, and fosters a culture of continuous improvement and high performance.
Additionally, the Senior Director oversees client placement and access to services, ensuring timely, equitable, and clinically appropriate service delivery across the network. This includes managing referral pathways, service availability, and coordination with provider partners to meet client needs.
The Senior Director partners closely with executive leadership to align clinical operations with organizational goals, regulatory standards, and industry best practices. This role also serves as a key representative of CCS in external engagements and regulatory communications.

Strategic Leadership & Oversight
- Lead and manage the specialized care and clinical case management teams, ensuring alignment with CCS’s strategic objectives.
- Develop and implement clinical strategies to improve care path adherence, client outcomes, and provider engagement.
- Provide clinical oversight for high-risk cases and escalations, ensuring timely and effective resolution.
Clinical Case Management & Care Coordination
- Oversee the Clinical Case Management Team, ensuring comprehensive assessment, planning, coordination, and evaluation of client care.
- Establish protocols for case reviews, risk stratification, and multidisciplinary collaboration.
- Monitor case management metrics and outcomes, driving improvements in service delivery and client satisfaction.
- Provide oversight of Care Coordinator team operations, driving process efficiency and accountability to ensure the accurate and timely processing of clinical reports and authorizations.
Client Placement & Access to Services
- Oversee client placement processes, ensuring timely and appropriate matching with providers based on clinical need, geography, and service availability.
- Manage referral pathways and ensure equitable access to services across the CCS network.
- Collaborate with provider partners to optimize capacity and reduce barriers to care.
- Monitor placement trends and access metrics, identifying gaps and implementing solutions.
Operational Excellence
- Optimize tools, workflows, and resources to support efficient and effective clinical operations.
- Analyze clinical trends and performance data to inform decision-making and strategic planning.
- Ensure compliance with all regulatory requirements and internal policies.
Provider Network Engagement
- Build and maintain strong relationships with CCS provider partners to support collaborative care delivery.
- Provide clinical consultation and support to providers, ensuring adherence to treatment guidelines and best practices.
Culture & Team Development
- Promote a person-first culture that prioritizes client needs and fosters respectful, constructive team dynamics.
- Support professional development and training for clinical staff, ensuring competency and confidence in service delivery.
- Lead staffing decisions, including hiring, performance management, and team assignments.
Escalation Management
- Implement proactive strategies to prevent and manage clinical escalations.
- Oversee resolution of complaints and grievances, ensuring timely and client-centered outcomes.
- Foster a culture of continuous improvement in escalation handling and service recovery.
Collaboration & Communication
- Partner with Clinical Excellence and other departments to develop and maintain treatment plan guidelines.
- Communicate industry trends and recommended enhancements to leadership.
- Represent CCS in regulatory inspections, stakeholder meetings, and cross-functional initiatives.
Other Duties
- Support implementation of care path modifications and new modalities.
- Complete additional responsibilities as assigned by the Vice President of Network Operations.
