Public Health Nurse II

Overview

The Vermont Department of Health has an opportunity for an enthusiastic and experienced nurse to join a dedicated and caring interdisciplinary team that will make a difference in the health of our community. We are seeking a well-organized and passionate nurse to complete our team of public health professionals in the Brattleboro Office of Local Health. The majority of the work will be in Family & Child Health but will also include immunization education and administration as well as other public health nursing duties. This is meaningful work with a prevention focus.

 

The role presents a unique opportunity to have a broad and meaningful impact on the health and well-being of specific populations. The position works with community partners and systems (including medical practices, state agencies, and other direct services) to promote improved population outcomes and primary prevention.  This is not a direct care/clinical care or individual case management position, rather, a public health-focused position that uses nursing skills to improve community health outcomes. 

 

The Family and Child Health (FCH) Coordinator aspect of this position focuses on increasing health equity for people of reproductive age, people who are pregnant and postpartum, infants, small children, and families. This work focuses on prevention with the potential to achieve long term, sustainable impacts on health by developing and maintaining relationships with the local systems of care for children and families. This also includes work intended to ensure that children in foster care have their medical needs met during their initial transition of caregivers.  Coordination and facilitation of community coalitions that engage care providers and community members on topics such as substance use in pregnancy, perinatal mental health, infant safe sleep, oral health, and breastfeeding/chestfeeding are also important parts of the role.

 

The job duties of the immunization aspect of this position include providing immunizations to community members who have limited access to care through monthly immunization clinics, outreaching marginalized populations, and identifying barriers to immunization access. The position also assures proper use and management of publicly funded vaccines at the district office, including educating other district staff about use and storage.

Environmental Factors

Duties primarily performed in a standard office setting. Work may be performed in clinic settings and possibly client homes. Exposure to communicable diseases and other stressful situations may occur. Private means of transportation is required. May be required to work evenings, weekends and 12 hour shifts.

Minimum Qualifications

Associate's degree in nursing AND five (5) years or more of professional nursing experience INCLUDING three (3) or more years in public health or community health nursing*.

OR

Bachelor of Science in Nursing (BSN) AND three (3) or more years in public health or community health nursing*.

Possession of, or eligibility for, licensure as a Registered Nurse (RN) in Vermont OR eligible to practice in the state of Vermont via a multi-state license.

*Community Health Nursing is defined as school nursing, home health nursing, home visiting nurses and community nurse health educators.
