Location: Beaver, PA
Reports to: Executive Director or Administrator

Position Summary
The LPN/RN Supervisor ensures high-quality, safe personal care by overseeing direct care workers (DCWs). This role includes DCW competency assessments, compliance with regulatory standards, client care coordination, and maintaining clinical oversight in non-medical home care.

What You Will Do:
Direct Care Oversight
•    Conduct annual competency reviews for all DCWs using observation, performance testing, and client feedback, as required by 28 Pa. Code Chapter 611.56.
•    Lead new hire orientation, including agency policies, charting, and client-centered care standards.
•    Monitor DCW performance and compliance with health screenings, background checks, and TB clearances—initiating corrective actions when needed.
Clinical & Quality Assurance
•    Serve as the first responder for client and caregiver concerns or care-related escalations.
•    Recommend and implement ongoing training programs for DCWs to maintain or elevate skill levels.
•    Facilitate liaison communication between DCWs, families, and executive leadership regarding care planning and staffing.
Documentation & Compliance
•    Ensure all personnel files are current, containing required documentation such as TB tests and background clearance records.
•    Assist the agency in maintaining compliance with licensing standards and survey readiness.

Required Qualifications
•    Active LPN or RN license in the Commonwealth of Pennsylvania.
•    Minimum 2 years of clinical nursing experience.
•    At least 1 year of supervisory or management experience, particularly in home care, personal care, or human services.
•    Strong organizational and communication skills; familiarity with home care operations is preferred.
•    Ability to analyze performance data and implement process improvements.
