Responsibilities

The Manager of Quality Management will take on the responsibility for the development, implementation and evaluation of an Evidence Based Clinical Practice Model which supports quality outcomes for Home Care.  In this role, you will implement the Evidence Based Clinical Practice Model and facilitate the development of standards of care, policies, procedures, protocols, and documentation guidelines consistent with the model.  You will collaborate in the development and dissemination of administrative and clinical policies, standards of care, policies, procedures, protocols, and documentation guidelines reflecting current research, institutional and industry standards.  You will work with the appropriate Quality Management staff to develop clinical performance indicators, manage the related analysis and benchmarking, and disseminate information.  You will identify and develop Performance Improvement initiatives to enhance processes that support the provision of high - quality care including the development and implementation of Performance Improvement Teams.  You will oversee participation in state-wide Performance Improvement initiatives and to ensure compliance with New York State Department of Health Regulations, The Joint Commission, Medicare Conditions of Participation, and other regulatory requirements by serving as a resource on regulatory matters and development of policies and procedures which reflect them.  You will manage the performance of direct and indirect reports and to provide orientation, coaching, training, and development when indicated as well as take corrective action when necessary.

Qualifications

  • Baccalaureate Degree Required
  • Master’s degree in a health-related field preferred.
  • NYS Licensed Registered Nurse, Physical Therapist, Occupational Therapist or Social Worker required.
  • Prior experience in Home Care and/or Long-Term Care Management or Quality Management/Performance Improvement, Team Facilitation, application of Statistical Processes Control Tools, Development of Policies, Procedures and Protocols.
  • NYS DOH Regulations and Joint Commission Standards Survey Processes.
