Position Description
The Anoka County Community Social Services Department is recruiting for a Temporary Social Worker in our Adult Mental Health service area. This social work/case manager position will provide adult mental health case management for a case load of adults with serious and persistent mental illness and their families. Many individuals served are under a civil commitment and the county is mandated to provide this service, as required by federal and state statutes, rules and/or health plans to adults who meet eligibility requirements. The primary objective is to coordinate access to services and resources specific to the adult’s mental health needs that allow the adult to remain in their home and/or community.

This is a temporary, exempt, hybrid position. Interviews will take place the week of January 20th, 2025 for those selected to move forward in the hiring process.
Pay & Benefits
Salary: $70,012 to $84,011 annually ($33.66 to $40.39 per hour).
2025 Anoka County Salary Schedule Grade 34: $70,012 to $98,009 annually ($33.66 to $47.12 per hour).

Work Location
This position will work at the Rum River Human Services Center, Building #9, 3300 4th Ave, Anoka, MN 55303.
This position may be eligible for flexible work arrangements, including hybrid work, with some days working remotely and some days working in the office.
Expected work hours are Monday – Friday, 8:00 a.m. – 4:30 p.m.
Job Duties and Responsibilities

Maintain caseload comprised of adults with serious and persistent mental illness with severe emotional disorders.
Assess plan, refer and monitor services designed to meet the individualized mental health needs of the children.
Support families as they strive to understand and meet the mental health needs of their adult family member.
Evaluate and screen maltreatment reports to determine need for Emergency Protective Services.
Identify people and communities in need of help. Evaluate clients (existing, new and prospective) based on their needs, limitations and desires.
Assess clients’ needs, situations, strengths, and support networks to determine their goals.
Build rapport with clients and their families.
Assist clients in receiving services by telephone and in person.
Respond to crisis situations such as child abuse and mental health emergencies.
Help clients adjust to changes and challenges in their lives.
Monitor clients' situations and follow up to ensure that they have improved.
Advocate or raise awareness with and on behalf of their clients.
Timely completion of paperwork and case notes in SSIS, maintain electronic files in OnBase, as well as computer usage necessary for other work duties.
Participate in individual and group clinical supervision in compliance with being an Adult Rule 79 Case Manager.
Provide leadership, coaching, and/or mentoring to a subordinate group as assigned.
Qualifications and Requirements:
Minimum Knowledge, Skills, and Abilities Needed
Bachelor’s degree in social work or a closely related field.
Two years (2,080 annual hours) of job-related experience working directly with adults with serious and persistent mental illness.
