Job Summary:
This position has clinical and operational responsibility for all Medical Fee for Service programs across all Weight Management Center sites. Facilitates coordination of treatment with specialty services (i.e. Diabetes and Endocrinology, Preventive Cardiology, Gastroenterology, etc.). Provides direct patient care in individual and group settings. Contributes to the development and completion of departmental goals and outcomes projects. Promotes the activities and development of the Weight Management Center through community outreach, patient engagement strategies, and relationship building with internal and external referral sources.

Education/Experience:
- Bachelor's degree with 5 years' experience in a healthcare setting.
- Master's degree preferred.
- Experience supporting the growth and development of program initiatives preferred.
ESSENTIAL FUNCTIONS:
- Responsible for planning, implementation, monitoring, and ongoing evaluation of medical fee for service programs to demonstrate operational effectiveness and desired outcomes.
- Develop and manage clinical protocols and workflows, program content, and curriculum
- Devise strategies for patient program engagement (e.g. support groups, cooking demo’s)
- Ensure program continuity and adherence to program standards and quality metrics
- Implement ALL program changes for the Weight Management Center at all sites, working with other teams as needed to ensure excellent patient experience
- Responsible for patient communication for all sites
- Monitor fee for service enrollment and participation at all locations (e.g. statistics to include number of new patients, number of active patients in program, etc.)
- Patient recruitment and enrollment strategies
- Manage patient pipeline
- Collaborate on new patient recruitment strategies and seminar content
- Ensure new patient slots are filled
- Ensure adequate number of patient slots by provider to minimize patient wait times
- Participates in performance improvement initiatives
- Facilitates collaboration and relationship building with internal partners such as nursing, physicians, and administration as well as external partners in the community.
2. Retail Management:
- Collaborate with program managers and program coordinators to maintain appropriate products and inventory for all sites
- Reporting of sales for all sites
3. People Management
- Performance management related to annual performance reviews (files, documentation)
- Input into recruitment, hiring, termination
- Staff scheduling and time/labor
- Assignment of tasks
- Facilitates collaboration and communication among interdisciplinary team to include fee for service specific team meetings and professional development
4. Direct Patient Care
- May participate in the patient evaluation process and provides education for the Weight Management Center
- Participates in interdisciplinary clinical programs to provide optimal care and improve patient outcomes
- Updating knowledge and skills to maintain, improve and document competency
- Contributing to the continuous improvement of patient outcomes and patient care services and the achievement of hospital and/or department goals
5. General Expectations
