What You Will Do
Deliver high-quality, evidence-based physical therapy treatments in patients’ homes
Implement individualized treatment plans developed by supervising physical therapists
Promote safety and functional independence in the home setting
Educate patients and caregivers on exercises, mobility, and home safety
Document patient progress in compliance with Vanderbilt Health and regulatory standards
Coordinate care with nurses, physicians, and allied health professionals
Travel to patient homes across designated service areas
Required Qualifications
Excellent communication and interpersonal skills
Ability to work independently and manage a flexible caseload
Active Physical Therapist Assistant (PTA) license in Tennessee
Minimum one year of experience as a Physical Therapist Assistant
Valid driver’s license and reliable transportation
Preferred Qualifications
Experience with electronic medical record systems
Strong understanding of rehabilitation techniques and functional mobility
Ability to work independently and manage a flexible caseload

KEY RESPONSIBILITIES
Provide treatment as specified on the written treatment plan by the supervising Physical Therapist. Demonstrates ability to:
Administer various therapeutic modalities according to departmental protocols and guidelines. These include ultrasound, whirlpool, T.E.N.S., biofeedback, hot/cold packs, paraffin baths, electric stimulation, weights, traction, massage and other as indicated.
Supervise patients in the completion of their exercise and training program to maximize their functional independence.
Progress the patient in treatment as indicated by the patient's progress in therapy and the treatment plan.
Respect the patient's right to privacy and confidentiality.
