CHJC is looking for a RN to join the team. The agency has a robust benefits package and a very desirable schedule. This is a salaried position for DAYS only.
Summary/Objective:
The role of the Registered Nurse (RN) is to provide quality patient care according to the Community Clinic of Jefferson County and New York State Office of Mental Health Standards of nursing practice Article 31 outpatient clinic, including administration of medications and treatments. RN must demonstrate ability to respond to medical and psychiatric emergencies in a prompt, precise and professional manner and be able to work in collaboration with other medical and non-medical professional staff to provide comprehensive, cost-effective care and to support a problem solving approach to issues. Assess, plan, document and evaluate patient care. Participates in quality improvement projects. Acts as a resource for nursing issues and keeps abreast of all aspects of assigned patient care. Contributes to multidisciplinary treatment planning by bringing a nursing focus to team collaboration and contributing to multidisciplinary treatment planning. Communicates effectively with medical team, Clinician team, Care Coordination Team, Crisis Team, and Clinic managers. Interacts effectively with patients, families, and significant others. Reports directly to the Practice Manager and will be assigned according to the needs of the clinic.
Essential Functions:
1. Assesses and triages patient’s physical and psychosocial needs.
2. Conducts health assessments and foster care treatment plans within the clinic or TCRP.
3. Prioritizes patient care by urgency of needs and acuity.
4. Collects critical information from available sources (i.e. hospital records, medication lists, other clinic records, lab results, etc.) and makes information available to the provider.
5. Formulates a nursing plan based on assessed needs.
6. Maintains up-to-date knowledge of nursing regulations and requirements in compliance with OMH standards: TCRP (OMH 589), Community Clinic (OMH 599/Article 31).
7. Administers medications and treatment as directed by the provider.
8. Reviews meditation books in the TCRP program to ensure medication pill packs are up to date and correct. Sign off on MAR.
9. Ability to transport TCRP residents to and from medical appointments and complete necessary paperwork.
10. Reviews and audits medication logs regularly within programs to ensure accuracy and compliance.
11. Conducts Health Monitoring appointments as required by NYS OMH.
12. Assists in completing Client treatment plans with the oversight of the Psychiatric Nurse Practitioners.
13. Implements the medical and nursing plan of care using the nursing assessment of the patient’s ability to comply, educational needs and preferences and any self-management goals agreed upon.
14. Documents and maintains appropriate records, tasking reminder follow-up in the Electronic Medical Record (EMR).
15. Works with the providers to plan course of each day.
16. Is knowledgeable of patient load and expectations, through meetings or individual planning meetings with providers, with exception to Walk-In staff where pre-planning is not feasible.
17. Provides leadership, direction, and acts as a resource for agency staff.
18. Assures that education given to patients is accurate, age appropriate, consistent, and fits the plan of care outlined.
19. Educates parents or foster parents on medications, treatment, or diagnosis as needed.
20. Demonstrates participation in professional development of self and team while always maintaining a positive attitude.
21. Assists the agency in adhering to all regulatory requirements and guidelines.
22. Assists with the Clinic, TCRP and/or Foster Care Quality Improvement process.
23. Attends necessary interdisciplinary meetings.
24. Stays up-to-date on current nursing practices and professional standards.
25. Manages Clinic Medical supply inventory.
26. Manages Clinic samples and discards as necessary.
27. Other duties as assigned by agency management.
Work Environment:
Work Environment: Office environment will require occasional travel to local schools, conferences and/or meetings and moderate travel within the community to include off-site/in-home visits. May have contact with service recipients that are agitated and/or are in crisis. Some risk involved working with service recipients with mental illness. Works in an office setting with a controlled temperature environment. Occasional exposure to inclement weather conditions may occur depending upon assignments.
