What You’ll Do:
- Travel to client homes and provide physical therapy services
- Develop functional goals and personalized recovery plans
- Coach patients and caregivers on exercises and modifications
- Communicate with nurses, case managers, and primary care providers
Requirements:
- MN Physical Therapy license in good standing
- Strong interpersonal and adaptability skills
- Valid driver’s license and reliable transportation
