?Position Summary:
The Wound, Ostomy, and Continence Nurse (WOCN) provides expert-level nursing care and clinical oversight for patients with complex wounds, ostomies, and continence issues in a Medicare-certified home health environment. This role supports high-quality, evidence-based care and ensures that all services meet Medicare Conditions of Participation, including documentation, OASIS accuracy, and coordination of care. The WOCN functions as a direct care provider, clinical educator, and resource to the interdisciplinary home health team.
Required Experience:

Current RN license in the state of practice

Graduate of an accredited nursing program.

Certification by the WOCNCB (CWOCN, CWCN, COCN, or CCCN).

At least one year of recent clinical experience in wound and/or ostomy care.

Valid driver’s license, car insurance, and reliable transportation.

Familiarity with CMS Home Health Conditions of Participation and Medicare documentation requirements.

Preferred two or more years of home health experience.

Preferred working knowledge of Home Health EMR systems (e.g., Homecare Homebase, WellSky, Axxess).

Preferred strong understanding of OASIS-E documentation and outcome-driven care models.
Primary Responsibilities:

Perform comprehensive assessments of wounds, ostomies, fistulas, pressure injuries, and incontinence within the scope of Medicare-certified home health practice.

Develop and oversee individualized care plans that comply with the CMS Conditions of Participation and are aligned with physician orders and patient goals.

Provide direct skilled nursing care, including advanced wound dressings, negative pressure wound therapy (NPWT), and ostomy site care and education.

Accurately complete and review OASIS documentation to ensure compliance with regulatory and reimbursement standards.

Collaborate with the interdisciplinary team—including RN case managers, PT/OT, speech, social workers, and physicians—to ensure coordinated, patient-centered care.

Educate home health clinicians, patients and caregivers on disease processes, self-care skills, wound prevention, and ostomy management to support safe discharge and reduce rehospitalization.

Monitor wound healing progression, document changes in condition, and update care plans and physician orders as necessary.

Recommend and coordinate wound and ostomy supply use in compliance with Medicare coverage criteria (LCD/NCD guidelines).

Participate in quality improvement activities and case conferences as a wound care subject matter expert.

Mentor staff nurses and support ongoing clinical training on wound and ostomy care practices and CMS do
