Job Summary:

The specialty APP in the ambulatory care setting works in collaboration with a multidisciplinary health care team. The specialty APP is generally service/physician aligned, managing a distinct patient population in the outpatient setting. The APP functioning primarily in a clinic setting may be required to perform some coordination of inpatient care. The specialty clinic APP has a strong foundation in evidenced based practice/education and is responsible for ensuring adherence to clinical pathways/protocols and ensures an environment of safe quality care. The APP demonstrates an advanced level of medical knowledge, clinical/technical competence, sound clinical judgment, professional communication skills, timely and compliant documentation, and responsible for professional development and competency validation.

Department Summary:

The General Neurology Division oversees a wide array of neurologic conditions, and is often the first point of contact for some of the most unique and complicated cases that walk in the door. As such, faculty and staff in this division are experienced to manage such cases, but also have access to collaborate with all other sub-specialties on a regular basis. Access to highly specialized professionals enables VUMC Neurology to provide an exceptionally high level of care for even the most challenging cases. In addition to expert consultation, access to VUMC’s testing and imaging facilities, clinical trials, treatment options, etc. benefit patients, trainees, faculty, collaborators, and researchers.

As one of the largest stroke facilities in the Southeast, Vanderbilt treats over 1,000 acute stroke victims each year. Named an Advanced Certification Comprehensive Stroke Center, Vanderbilt is located in the heart of the stroke belt, that is, a southeastern region of the U.S. that public health officials have recognized as having an unusually high number of stroke victims. This high volume of patients is met with a large faculty, providing a breadth of expertise to patient care and research. Patients are supported by an interdisciplinary care team equipped with the latest advances in stroke medicine, including intravenous thrombolysis and mechanical thrombectomy. Stroke care at Vanderbilt goes beyond acute clinical treatment—Vanderbilt neurologists collaborate with physicians and researchers across the institution to provide prevention education, follow-up care and rehabilitation.

KEY RESPONSIBILITIES

• Assessment of Health Status
• Performs and documents in the medical record a complete history and physical examination for acute and complex chronically ill patients.
• Orders and collects data using appropriate assessment techniques, relevant supporting diagnostic information and diagnostic procedures where indicated.
• The nurse practitioner may serve as the first responder for both nursing and house staff in the event of patient status changes, orders, and in situations requiring procedural and resuscitative interventions.
• Assesses for risks associated with the care of the acute and complex chronically ill patient including medication side effects, immobility, impaired nutrition, fluid and electrolyte imbalance, immunocompetence, invasive interventions and diagnostic procedures.
• Coordinates daily clinic schedule with the attending physician team, presents patient report, collaborates in plan of care, and follows assigned patient population.
