Position: Licensed Vocational Nurse (LVN) – Home HealthLocation: Modesto, CaliforniaCompensation: Starting at $40/hour, depending on experience — open to negotiation!
What You'll Do:- Deliver direct patient care in a home health setting according to individualized care plans.- Monitor patients’ health status, including vital signs, medication administration, and timely reporting of any changes to the supervising RN.- Provide patient and family education on treatment plans and health management to foster independence.- Assist with wound care, catheter care, and other skilled nursing tasks as needed.- Collaborate closely with the interdisciplinary care team to ensure safe, coordinated, and effective care.- Accurately document all services and patient interactions in a timely manner.
What You'll Bring:- Current California LVN license- At least one (1) year of LVN experience, preferably in home health or a similar setting- CPR certification- Valid driver’s license and reliable, insured transportation
