Job Description Summary

‎ 

The Population Health Analyst is a Social Worker is primarily responsible for assessing the psycho/social needs of patients/families/caregivers, educating patients, families, and Millennium employees in understanding what type of community resources are available and coordinating with the appropriate resources to assist the patient. Resources include but are not limited to food, shelter, utilities, pharmaceutical financial needs. S/he must exhibit strong interpersonal and communication skills as well as the ability to utilize both internal and external resources to accomplish the goals and objectives.

Essential Duties and Responsibilities include the following. Other duties may be assigned. 

  • Determines nature of patient’s situation by interviewing, assessing the medical, psychological, emotional and social information to assist with patient needs; 

  • Research, and gather information regarding community resources, including pharmaceutical resources for patients needing assistance in paying for copayments; 

  • Assist with patient placement to Skilled Nursing Facilities; 

  • Support Millennium’s employees in the coordination of the community resources to effective support the patient’s care plan; and 

  • Meet with community resources within the designated area to build relationships to assist patients in obtaining those resources;  

  • Maintains client confidence and protects operations by keeping information confidential; 

  • Travel required, in home assessments of Social Determinants of Health, assist patients with completing applications.
