As an RN Specialist, you will be responsible for planning, coordinating and implementing all aspects of therapy for cardiac and pulmonary patients. Cardiopulmonary Rehabilitation prevents readmissions and cares for complex patients with cardiac problems and disease, such as valve replacements, CABG, stents, MI, heart failure, PAD and pulmonary disease. This role supports Cleveland Clinic's mission and vision by
providing high-quality, patient-centered care that improves the health and well-being of people with heart conditions.
A caregiver in this position works 1 -2 shifts per month, days from 6:45AM—6:45PM
A caregiver who excels in this role will:
Evaluate patient's clinical status and provide recommended rehab services as per protocol.
Utilize assessment skills in providing patient care in all phases of cardiac rehabilitation.
Educate patients/families about cardiopulmonary diseases and treatments.
Participate in case management and discharge planning conferences with other interdisciplinary staff.
Document patient progress and collaborate with various members of the healthcare team to meet patient needs.
Develop rehabilitative goals to prevent progression of heart and lung disease (exercise prescription, activities of daily living, risk factor reduction, prevention of exacerbations, etc.).
Assist in coordinating patient care and developing new programs, educational materials and policies/procedures.
Assist in developing and evaluating performance improvement standards.
Remain flexible working between all phases of rehabilitation, cardiopulmonary procedures and as indicated in the Heart Center.
Attend continuing education programs to facilitate professional growth.
Support and participate in community wellness programs.
