POSITION SUMMARY: The Medical Specialist Liaison is responsible for developing and maintaining positive, effective relationships between Rocky Mountain (RM) PACE and our contracted medical specialty partners across our service area. This position’s main objective is to build positive specialist experience with RM PACE which translates into the best care for our participants. She/he will serve as an effective intermediary between the participant, the specialty practice and various RMHCS departments. The Medical Specialist Liaison is heavily involved in quality improvement efforts.

RESPONSIBILITIES AND DUTIES:
ESSENTIAL JOB FUNCTIONS:
- Develops and sustains professional relationships with contracted medical specialty partners and various RMHCS departments.
- Meets with contracted medical specialty partners for periodic review of PACE coordination with their practice/operation.
- Identifies, analyzes and drives resolution of partner concerns and process improvement opportunities.
- Facilitate/coordinate PACE stakeholder involvement towards resolution of specialty partner concerns. Will collaborate with managers, directors and executive team members (ie. CMO, VP Nursing, VP PACE)
- Organizes and documents meeting minutes and follow-up for all improvement processes.
- Point person for complaints from participants, RMHCS staff and contracted medical specialty partners related to medical specialty services.
- Must use personal judgement with little to no oversight in providing/pursuing resolutions to issues encountered.
- Works independently in the field and leads a multi-disciplinary approach to resolutions through project management.
- Works with PACE contracts department to monitor contracted medical specialty partner relationships and adequate supply of specialty partners for the PACE population.
- May assist in gathering and collecting data related to medical specialty partners- will work closely with Quality department and Data Analyst/Assistant COO
- Strong/broad understanding of PACE processes, including clinical, home care, scheduling, transportation, and medical records.
- Able to navigate electronic medical record.
- Other duties as requested/assigned.
