Position Summary:

The Senior Manager, Site of Care Strategy, is responsible for leading the design, build, and implementation of strategic programs that optimize care delivery across diverse healthcare settings. This role drives the development of comprehensive Logic Models, Action Plans, and measurable Objectives and Key Results (OKRs) to ensure alignment with organizational goals and performance outcomes. The Senior Manager will collaborate cross-functionally to translate strategic vision into operational execution, leveraging data-driven insights to inform program development, monitor progress, and continuously improve site-of-care initiatives. Success in this role requires a strong ability to manage complex projects, influence stakeholders, and deliver scalable solutions that enhance patient access, quality, and efficiency. 


Key Roles and Responsibilities:

  • Collaborate with ACO partners to design, implement, and refine care pathways that improve patient outcomes and reduce unnecessary utilization. 
  • Analyze post-acute care trends, SNF performance metrics, and readmission rates to identify opportunities for improvement. 
  • Lead cross-functional teams in developing strategic initiatives that align with ACO goals and CMS benchmarks. 
  • Analyze and report on key performance indicators (KPIs) related to post-acute spend and care transitions. 
  • Facilitate stakeholder engagement, including clinical teams, care coordinators, and external providers, to ensure alignment and accountability. 
  • Lead in the development of training materials, workflows, and toolkits to support care pathway adoption. 
  • Leverage data analytics and visualization tools to communicate insights and drive strategic planning. 
  • Stay current on regulatory changes, CMS guidelines, and industry best practices related to value-based care and post-acute management. 
  • Continuous focus to improve operational processes and methods and implement Operational Plans to fulfill organizational goals and mission. 
  • Collaborate with PS and CL to ensure that operational interventions are aligned with overall partner strategy
  • Consume data trends from monthly claims files for assigned partners using Next Site of Care Dashboard 
  • Compare current data with KPIs to determine any course corrections needed (i.e. action plans)
  • Incorporate data findings into action planning meetings and report out on any care redesign initiatives in progress along with recommendations for other care redesign efforts supported by the data 
  • Review and prepare SOC action slides for Action Planning and JOC meetings
  • Work with Partner Success and Clinical Consultants to determine Acute Care Redesign activities for success (both System level and at the individual acute care hospital level)
  • Strategize with Partner Success and partner leadership on engagement of existing teams and processes that will facilitate the smooth implementation of care redesign
  • Work with the local system and individual hospital leadership to identify resources to ensure success in site of care program and outline roles and responsibilities for teams and contributors.
  • Streamline site of care workflows to ensure succession planning in a dynamic system (capture individual knowledge to maintain care redesign momentum in the event of personnel changes at the system or individual hospital level)
  • Maintain good working partnership with workgroup leaders at partner sites
  • Additional responsibilities as the client’s site of care program matures and focus shifts from implementation to full program operations
  • Collaborate with post-acute Operations team.
  • Readmission root cause analysis meetings with hospital system
  • Collaborate with hospital system to host quarterly PAC town hall
  • Annual SNF and HHA Performance Network review/refresh and associated patient choice list updates
Requirements

Minimum Qualifications:

  • Bachelor's degree in healthcare administration, Public Health, Business, or related field. 
  • Clinical License preferred 
  • 7+ years of experience in healthcare operations, care coordination, or value-based care programs. 
  • Strong analytical skills with experience in healthcare data analysis and reporting. 
  • Excellent communication and project management skills. 
  • Familiarity with ACO models, SNF operations, and post-acute care strategies. 
  • Proficiency in Microsoft Excel, PowerPoint, and data visualization tools (e.g., Tableau, Power BI).
