CORA Services is seeking a Community Case Manager to support the Community Schools team at George Washington High School (19116 zip code).   

The Community School Case Manager is responsible for providing integrated case management services to young people and their families at an assigned community school. The Community School Case Manager will carry a caseload and work to help families increase protective factors, increase family functioning, increase family engagement in school activities and increase student's attitudinal connectivity and relationship to school, which, in turn, will increase student attendance and achievement. 

 

Rate:  $21.43/hour 

(Rate commensurate with experience and credentialing
 

Responsibilities:

  • Uphold the mission, vision, and values of CORA Services, which includes valuing all individuals, supporting the individual’s potential for growth, and enhancing a pragmatic service system approach.
  • Collaboratively support schools, students and families to provide resources and linkage to services for students and families experiencing socioeconomic or health stressors.
  • Maintain a caseload size twenty-five cases or more.
  • Assist students and families increase protective factors, increase family functioning, increase family engagement in school activities and increase student's attitudinal connectivity and relationship to school, through the planning, coordination, and implementation of Family Forums on a monthly and/or quarterly basis.
  • Engage with students/families in a collaborative relationship that will enhance the potential for the client’s completion of service with positive outcomes.
  • Involve families at the school and help navigate school-based resources to increase parent engagement at the school and community levels.
  • Conduct regular home visits, as needed to include assessing the families’ needs for services and developing a service plan to remove barriers to regular school attendance.
  • Complete weekly contact with families on assigned caseload to support with social services and health navigation as needed.
  • Assist the Attendance Case Manager, by attending Student Attendance Improvement Conferences, when necessary, and working with school’s attendance team to ensure barriers to regular school attendance are removed for families experiencing truancy
  • Plan, coordinate, and implement weekly, monthly, and quarterly attendance incentives and events in partnership and collaboration with the school attendance team and CORA case management leadership team.
  • Comprehensively complete referrals and linkages to services and supports to remove any barriers a student or family face to equitably accessing a program/resource.
  • Support families to establish or maintain household stability and submit request for the use of emergency funds assistance as applicable
  • Attend collaboration and attendance team meetings as scheduled to support the school with proactive development and implementation of a school-wide Incentive Attendance Plan
  • Participate in School District of Philadelphia (SDP)’s Supportive Team or Multi-Tiered System meetings of supports and work closely with Schools, Community School Coordinators/Managers and other School Partners.
  • Develop a plan of service for each child assigned to caseload that could include enrollment in services such as Out-of-School Time and provide them with the information, tools, school supplies, uniforms, and other resources to attend school regularly.
  • Adhere to training requirements established by Agency or other regulatory entities.
  • Attend regular supervision and department meetings.
  • Perform other duties as assigned by the Community School Case Management Supervisor and Director, as needed.

Educational Requirements:

  • Bachelor or Master level degree in a Social Services field (ex. Social Work, Counseling, Criminal Justice or closely related field)
