As Care Manager LVN, you'll establish relationships and foster respect and rapport with our Health Management Operations and Health Value Teams and will work closely with our Provider Partners from the health care team responsible for the management and coordination for our hospitalized members at risk, and at their most vulnerable. We seek individuals with a strong commitment to patient-centered care and a passion for promoting health, wellness, and patient as well as provider experience.
You will focus on:
Conducting Initial and Concurrent Review and Discharge Planning/Care Coordination, utilizing evidence-based criteria coupled with your clinical expertise and collaboration with our Medical Directors to ensure appropriate admissions, level of care and timely discharges.
Monitoring and continually assessing and evaluating member severity of illness, intensity of services and response to treatment, making recommendations for appropriate continued stay, transition to lower level/tertiary care and coordinate member transitions.
Applying clinical knowledge, critical thinking skills, as well as your solutions-oriented problem-solving and strong decision-making approaches.
This position is a field-based opportunity in the San Francisco Bay area.
The Skills, Experience & Education You Bring
Current unrestricted California LVN licensure required
Minimum of 3-5 years recent acute/hospital clinical experience.
Computer skills proficient, experience in EMR, Microsoft Office Suite.
Desired experience in managed care concepts and practice.
The Base Salary for this position is $41.00 - $52.00/hr
