Positive Impact Health Centers (PIHC) is a community leader in providing HIV prevention, care and treatment services. The PIHC model of care assures that persons with HIV have access to medical, pharmacy, dental, behavioral health and social services, providing the best opportunity for patients to achieve high-quality health outcomes.

JOB SUMMARY: This hybrid position develops and implements a coordinated HIV outreach plan that advances

Prevention Services goals across Metro Atlanta, including virtual engagement. The role blends

community engagement and clinical prevention by providing HIV/STI testing, counseling,

condom distribution, and linkage to essential care and prevention services. It also supports

rapid entry to HIV care and the PIHC PrEP program through education, same-day linkage, and

ongoing client retention and re-engagement. In partnership with community organizations, the

position expands access to services and promotes awareness of HIV prevention across diverse

populations. All services are documented accurately, including timely partner notifications to

local health departments, and the role reports directly to the Prevention Program Manager. The

standard schedule for this position is Monday, Thursday, and Friday from 8:30 a.m. to 5:00 p.m., with

extended hours on Tuesdays and Wednesdays. Occasional evening and weekend hours are required to

support scheduled outreach events and community activations.

ESSENTIAL FUNCTIONS:

Duties and Responsibilities:

• Prepare HIV prevention outreach plan for Decatur and Duluth Prevention services and

update it annually or as directed.

• Act as liaison and first point of contact for event creation and scheduling

• Lead and/ or supports the organization of special events as assigned

• Research, scout, and generate new avenues of outreach and initiate and maintain

partnerships

• Act as primary outreach tester and representative of Positive Impact Health Centers and

its Prevention Services

• Network with other agencies, funders, and other organizations to promote Positive

Impact Health Center services and Prevention related outreach events

• Maintains the outreach supply closet by ensuring it is consistently clean, organized, and

fully stocked

Summary of General Duties, Tasks, and Responsibilities:

• Conducts HIV Counseling (CTL) sessions in a non-judgmental manner and with fidelity to the

core elements and core components of the HIV Prevention Counseling standards.

• Conducts offsite testing in a sex positive manner in non-traditional settings during nontraditional

work hours.

• Provide effective condom education and distribute condoms throughout the community

• Provide education on Pre-exposure prophylaxis (PrEP) and ensures people eligible for

PrEP if interested are linked to PrEP.

• Coordinate enrollment in PIHC’s Rapid Entry Program for individuals that are newly or

previously diagnosed with HIV

• Schedule medical appointments utilizing an Electronic Medical Record (EMR) system

• Performs other duties as assigned
