Community Palliative Care & Geriatric Care Program
Responsibilities
Provides nursing support working closely with medical providers,
Provides education, ongoing assessment and support to patients and families
New patient visit and intake .
Triage with patients and family for ll related to Community Palliative Care and Geriatric Memory Assessment Clinic
Liaison with PCP offices, LTC facilities, follow up
Required Work Experience:
Massachusetts license as an RN and BLS CPR certified.
Minimum of 3 years of nursing experience
Experience providing clinical care and triage in an outpatient setting
Demonstrated interest to work with patients population related to the specialty
Excellent verbal and written communication and interpersonal skills.
Must have competency in delivering exceptional nursing care and have proven success in partnering with both physicians, patients and family members.
Schedule and Location
Location - 21 Dwight Road , Longmeadow
Full Time 40 hours - No Weekends No Holidays
Monday- Friday 8-4:30, or 8:30-5 or 9-5:30
