Community Palliative Care & Geriatric Care Program

Responsibilities                         

• Provides nursing support working closely with medical providers,

• Provides education, ongoing assessment and support to patients and families

• New patient visit and intake .

• Triage with patients and family for ll related to Community Palliative Care and Geriatric Memory Assessment Clinic 

• Liaison with PCP offices, LTC facilities, follow up

Required Work Experience: 

• Massachusetts license as an RN and BLS CPR certified.

• Minimum of 3 years of nursing experience

• Experience providing clinical care and triage in an outpatient setting

• Demonstrated interest to work with patients population related to the specialty

• Excellent verbal and written communication and interpersonal skills.

• Must have competency in delivering exceptional nursing care and have proven success in partnering with both physicians, patients and family members.

Schedule and Location

• Location - 21 Dwight Road , Longmeadow

• Full Time 40 hours - No Weekends No Holidays

• Monday- Friday 8-4:30, or 8:30-5 or 9-5:30

• Sign On Bonus
