As a Community Health Promoter II, you'll create a positive Scripps Health experience for our patients while being responsible for the following:

  • Identify patients from the emergency department who qualify for Enhanced Case Management
  • Connect patients to Enhanced Case Management at local partner sites
  • Provide patient support and education which will be offered post discharge to link patients to needed social and medical supportive services.
  • Manage documentation as well as maintaining a list of community contacts and referrals
  • Implement community outreach and facilitate a variety of community-based education.
  • Support a variety of community-based interventions.
  • Assist in developing educational content.
 

Qualifications

 

Required Qualifications:

  • High school diploma or equivalent.
  • Excellent communication skills. Demonstrate oral and written communication skills by clear and accurate correspondence, including spelling and grammar usage.
  • Demonstrate experience and ability to work independently and as a team player. Ability to set priorities and goals without specific direction. Ability to handle problems, propose solutions, and implement them.
  • 1-2 years community-based work experience.
  • Verbal and oral presentation skills.


Preferred Qualifications:

  • Bilingual in Spanish.
  • Basic typing and computer skills.
