THIS IS A GRANT FUNDED POSITION

 

JOB SUMMARY/ESSENTIAL JOB FUNCTIONS: The Intervention-Outreach Assistant works in the Center for Youth Advocacy and Well-Being (CYAW). The Center for Youth Advocacy and Well-Being works to improve the health and well-being of youth and their families so that they may thrive in their homes and communities. This position assists youth and families to coordinate delivery of health, wellness, and mental health services by helping them navigate child-serving, educational, and human/social services systems. This position utilizes the wraparound approach to promote a healthier youth and family environment to mitigate the Adverse Childhood Experiences (ACEs) and trauma.

 DUTIES AND RESPONSIBILITIES:

  1. Partners with families to develop an individualized care plan that is based on information collected through the screening and family interviews to address medical, social, educational, and behavioral health needs to further family goals, plans, and success strategies.
  2. Advocates for youth and families with behavioral health, educational, and community partners.
  3. Coordinates referrals to services and related agencies to meet the needs identified during screening.
  4. Assists with identifying and establishing natural, community, and professional supports to aid the youth and family in thriving in their communities.
  5. Assists with educational sessions and training for families, community partners, and UTHSC staff.
  6. Completes referrals and enters notes in the case management system.
  7. Provides follow-up to the youth, families, and agencies.
  8. Adheres to HIPAA guidelines and maintains confidentiality.
  9. Performs other related duties as assigned.
 

Qualifications

 

MINIMUM REQUIREMENTS:

 EDUCATION: High School Diploma or GED. (TRANSCRIPT REQUIRED)

 EXPERIENCE: Two (2) years of related experience; OR Associate’s Degree in lieu of experience. Must have a valid Driver’s License and vehicle insurance. 

KNOWLEDGE, SKILLS, AND ABILITIES:

  • Knowledge of resources in the community to assist youth and families get connected with providers in the child and family serving systems.
  • Knowledge and experience in fostering community partnerships and collaborations.
  • Skills working with youth and families who have behavioral health concerns.
  • Ability to relate to youth and families.
  • Ability to maintain confidential information.
  • Ability to be self-motivated, maintain a friendly demeanor and exhibit tact and professionalism both in person and on the phone.
