Kitsap Mental Health Services (KMHS) is seeking an ECS Contract Clinician to join our multidisciplinary team supporting individuals and their caregivers through specialized outreach-based services. Our team collaborates to provide respectful, compassionate, and individualized care to clients in community settings. As part of our team, you’ll contribute by planning, developing, and coordinating treatment, rehabilitation, and support services while providing behavioral education, skill-building, and consultation to caregivers, guardians, and adult family home providers. In this role, you will play a key part in helping clients achieve stability and improved quality of life while fulfilling Expanded Community Services Contract (ECS) requirements.
Primary Responsibilities:
- Maintain regular visits to ECS clients residing in designated adult family homes while providing behavioral support to caregivers and counseling to clients as specified in Cares Assessment and ongoing presenting behavioral needs arise to maintain adult family home placement.
- Assume primary responsibility for developing, writing, implementing, evaluating, and revising overall treatment goals and corresponding behavioral support plans in collaboration with the Home and Community assessment and Kitsap Mental Health treatment team for each client.
- Provide appropriate instructions, psychoeducation, and applicable techniques to adult family home caregivers to handle difficult situations and problem behaviors.
- Be available to consult with caregivers, families, and community agencies such as DSHS, court systems, housing authorities, etc., to maintain coordination in the treatment process.
- Document client progress to maintain a permanent record of client activity according to established policies and procedures identified in the ECS contract. Provide and maintain ECS Support and Guidance binder in the home for clients and their caregivers.
- Coordinate and participate in monthly staffing and treatment planning review meetings in the adult family homes with homeowner, client, and assigned ECS social worker. Monitor client progress and level of behavioral support need to assess need for continued ECS services. Document progress and collaboration regarding need in client chart.
- Track any emergency situations or significant changes in client presentation and communicate them with Home and Community social worker. In the event of a client’s hospitalization, notify Home and Community services within 24 hours.
- Provide and coordinate outreach and crisis intervention services to KMHS clients in the community and routinely identify and evaluate physical risk factors for self and others while in potentially unsafe neighborhoods, homes, while around people with unknown potential for aggression, and while in proximity of potentially unsafe and/or unclean structures and equipment. Consult with DMHP's and law enforcement officials when appropriate.
- Work in a cooperative and collaborative manner as a team member. Assist in building positive working relationships with staff of all agency departments.
- Organize and prioritize outreach activities appropriately and consistently. Work independently to meet documentation requirements.
- Pursue professional growth by seeking out learning opportunities and integrating new learning into daily work practices.
