Are you a Registered Nurse (RN) looking for a meaningful role with a flexible schedule and the autonomy to make a real impact? BAYADA @ Bayhealth is hiring a compassionate Home Health Admissions RN to join our team, providing one-on-one care (Admissions, Assessments, etc) to adult and geriatric clients throughout Sussex County.

As an Admissions RN, you’ll be the first point of contact for our new clients—conducting start-of-care visits, comprehensive assessments, and building strong clinical foundations for ongoing care in the comfort of patients’ homes.

Responsibilities:

• Perform start-of-care visits and comprehensive assessments (including OASIS)

• Develop and initiate individualized care plans

• Educate patients and families on the care process and expectations

• Collaborate with a supportive interdisciplinary team

Requirements:

• Active RN license in Delaware

• At least one year of clinical experience 

• Strong assessment and documentation skills

• Reliable transportation and ability to travel within Sussex County
