About the Department

The Chicago Center of HIV Elimination, embedded within the University of Chicago's main campus in Hyde Park, is located in Chicago's HIV epicenter. Of the 20 neighborhoods in the city with high annual HIV diagnosis rates (greater than 40 per 100,000), 13 of them are no more than eight miles from the University of Chicago. This area includes 4 of the 6 communities with the highest annual diagnosis rates in the city (Washington Park, West Englewood, Greater Grand Crossing and South Shore). Our position within the hardest hit neighborhoods in Chicago provides unique opportunities to advance HIV testing and prevention interventions locally, providing tangible results to those most affected and to improve the lives of those living with and without HIV infection.

CCHE seeks to eliminate new HIV transmission events over the next 30 years (from 2011 to 2041) by using network science to target and integrate prevention as well as create structural and community-specific interventions.

Job Summary

In this role, the Assistant Care Manager (ACM) is responsible for completing daily or shift specific quality audits, facilitating monthly team meetings, and directing delivery of quality, patient-centered care for individuals seeking linkage to care and supportive services, particularly therapeutic services. This is in collaboration with and under the supervision of the Director of the Population Centered Health Homes (PCHH). In the absence of the team’s Director, the ACM assumes responsibility for the Supportive Services unit.

This position is grant funded.

Responsibilities

• Training and supporting the Supportive Services team members, this includes team trainings on updated protocols and services, trauma informed care, de-escalation, etc.

• Complete regular audits ensuring documentation is up-to-date for the Supportive Services team.

• Provide task management and offer supervision meetings as needed to discuss clinical concerns.

• Hold a caseload of up to 20 mental health clients, providing 1:1 counseling services, completing notes, etc.

• Follow fidelity monitoring protocols in data collection and documentation in Apricot.

• Support clinical consultations and crisis intervention (internally referred to as Manager on Duty coverage) in order to support staff and clients when a crisis occurs.

• Availability to work weekends and evenings as needed.

• Applies research principles and relevant subject matter knowledge relevant to administer a research project. With a moderate level of direction, manages lab and/or research-related duties and tasks. Helps develop, design and conduct research projects according to plan.

• Supports data collection and analytical needs of research projects. Conducts literature reviews and helps write reports and manuscripts. Ensures project compliance with different policies, procedures, directives, and mandates.

• Performs other related work as needed.

Minimum Qualifications

Education:

Minimum requirements include a college or university degree in related field.

Work Experience:

Minimum requirements include knowledge and skills developed through 2-5 years of work experience in a related job discipline.

• Advanced degree.

Experience:

• Experience working with and/or providing care to LGBT populations, Black and Latino community members.

• Experience providing services for people with HIV and/or HCV.
