Job Summary

The Senior Program Manager, Community Health Improvement plays a key role in advancing Mass General Brigham’s Community Health Strategy by overseeing a team of 16 Community Health Workers/Resource Navigators embedded in priority communities. This role focuses on implementing place-based strategies tied to the identified health conditions.

Reporting to the Director of Community Health Programs, and aligned with Primary Care leadership, the Senior Program Manager ensures consistent practices across sites, supports staff development, and builds partnerships with clinical and community stakeholders. The role helps align frontline work with MGB’s Community Health Improvement Plan and systemwide efforts to improve health outcomes.

Principle Duties and Responsibilities

• Provide leadership, supervision, and ongoing development to a team of (16 Community Health Workers) across priority communities

• Oversee the day-to-day operations and coordination of outreach, education, and navigation services related to colorectal cancer and cardiometabolic disease

• Develop and implement clear workflows, referral processes, and standards of practice to ensure consistency across all CHW sites

• Leverage support from and collaborate closely with Clinical Community Program Leadership, including Primary Care and Community Health clinical partners, to align program operations with clinical workflows and improve care coordination

• Build and maintain strong relationships with clinical teams, local health centers, and public health agencies to support care coordination and access to screening and treatment

• Ensure all strategies are informed by CHNA/CHIP findings and are responsive to local community needs

• Partner with data and evaluation teams to monitor program impact, track outcomes, and inform continuous improvement

• Identify and facilitate training opportunities to support professional development and role clarity for CHW/Resource Navigator staff

• Represent the program at system-wide meetings, community coalitions, and other forums as needed

• Manage programmatic budgets and contribute to grant reporting and compliance activities

• Promote collaboration and alignment between clinical and community-facing teams

• Support the use of lived experience and community voice to inform strategy and program design

• Other responsibilities assigned in support of advancing community health and equity

• Direct supervision of Community Health Workers/Resources Navigator Health Corp


 

Qualifications

Education

  • Bachelor's Degree Related Field of Study or equivalent years of experience required and Master's degree in Social Work, Public Health, or related field of study strongly preferred.

Experience

  • 5 - 7 years’ experience in public health, community health, community engagement, not-for-profit, or health and human services.
  • At least 5 years of experience managing community-based programs and personnel required.
  • Experience conducting health promotion activities including screenings, navigation, education, and relationship building required.
  • Experience with program budgeting and financial management preferred

Knowledge, Skills and Abilities

  • Demonstrated experience cultivating, managing, and maintaining complex community relationships with a diverse set of internal and external stakeholders.
  • Experience working in close partnership with evaluation, operational, clinical, and administrative leadership to identify goals and create pathways to achieve the goals.
  • Experience utilizing data as a tool to evaluate performance and identify improvement opportunities.
  • Excellent written and verbal communications skills and the ability to tailor messaging to meet the needs of the audience.
  • Experience planning and hosting community events, listening sessions, trainings, and other community-based convenings.
