The Team

The Residential Aged Care Home (RACH) Liaison team sits within the Integrated Operations Centre (IOC) at the Royal Hobart Hospital and supports safe, timely and well‑coordinated transitions between hospital and residential aged care. The team works closely with patients, families, inpatient units, multidisciplinary teams, GPs and private RACH providers to ensure older Tasmanians receive care in the most appropriate environment.

As part of the Department of Health, the team helps improve patient flow, strengthen discharge planning and build strong connections between acute and aged care services. We work collaboratively to reduce delays, support continuity of care and guide patients and families through the RACH pathway, contributing to better outcomes and a more connected health system for the Tasmanian community.

The Role

The Clinical Nurse Consultant – RACH Liaison Nurse provides expert clinical leadership and coordination to support safe transfers between hospital and residential aged care. The role includes comprehensive assessment, care coordination and discharge planning to ensure patients’ needs are met in the right setting.

Key responsibilities include:

• Providing specialist advice to clinicians, patients, families and RACH providers on transfer processes, care type changes, non‑acute fees and complex discharge needs.
• Building strong relationships with RACHs, GPs, inpatient teams and other stakeholders to support seamless communication and continuity of care.
• Educating hospital staff on RACH pathways, operational requirements and best‑practice transfer processes.
• Acting as a central point of contact between acute services and RACH providers, including during periods of high operational demand.
• Leading quality improvement initiatives, contributing to policy development and supporting evidence‑based models of care.
• Working autonomously within the IOC while collaborating with the broader interdisciplinary team to improve patient flow and system performance.

This role is essential in ensuring older Tasmanians experience safe, coordinated and compassionate transitions into residential aged care, supporting the Department’s commitment to high‑quality, person‑centred care.

You will need:

• Evidence of well-developed clinical management and leadership skills with the ability to collaborate effectively with the interdisciplinary team to provide a versatile, flexible service which enables care needs to be met in the most appropriate environment.
• Demonstrated high level interpersonal and communication skills (oral and written) including mediation and conflict resolution skills, with proven ability to manage effectively within a diverse health care environment.
• Demonstrated effective organisational skills, highly developed reasoning/problem solving skills and apply principles of clinical risk management, quality improvement and evidence-based practice to the clinical setting and make independent and collaborative judgments.

Details of appointment:

Multiple, fixed term, full time, day worker positions, working 76 hours per fortnight, commencing as soon as possible for a period up to eighteen (18) months.

Salary: $118,297 to $125,543 per annum. Our Employer 12% superannuation contribution is on top of this amount.

• Salary range is in accordance with Nurses and Midwives (Tasmania State Service) Agreement 2023.

In addition, the following allowances calculated on the salaried incremental point may be available: 

• Post Graduate Allowance of up to 10% based on qualification type
