Job Summary
Summary:The Clinical Liaison Lead coordinates and leads the activities of the liaison team under the supervision and in collaboration with the Admitting Director or Regional Director.
The liaison coordinates the referral process to ensure a smooth transition of patients to Mass General Brigham Home Care.
The Liaison ensures the patient meets criteria for admission, is clinically appropriate, and is ready to transfer to a post-acute provider.
Perform preadmission patient evaluations when required.

Essential Functions:
Orient and mentor new team members thoroughly in liaison processes.
-Monitor staff activity and provide feedback. Identify efficiencies to ensure is monitored, feedback provided, and efficiencies identified to ensure admission goals are met.
-Assist with managing and assessing the timely clearance of insurance claims with appropriate escalation as needed.
-Patient assessment information is obtained from a variety of sources, and data is transmitted in appropriate systems (4Next/Epic and other applications).
-Trends or barriers to achieving established admission targets are identified and communicated to management.
-Collaborative relationships are developed and maintained with internal and external staff to ensure customer satisfaction.
-Referral source relationships are nurtured to ensure and maintain customer satisfaction.
-Challenging transfers are supported through SR/MGB HC clinical and supervisory resources.
-Discharge plan alternatives are considered to determine the most suitable plan to meet the patient’s needs.
-Coordinate staffing assignments and reallocate resources to meet the facility's needs.

Social Worker [Licensed Social Worker - LSW - State License] - Generic - HR Only preferred Physical Therapist [State License] - Generic - HR Only preferred Speech-language Pathologist [State License] - Generic - HR Only preferred Occupational Therapist [State License] - Generic - HR Only preferred
Experience
Broad knowledge of various insurances and government reimbursements 0-1 year required
Knowledge, Skills and Abilities
- Background knowledge/work with a clinically complex patient population.
- Excellent computer skills with working knowledge of Microsoft Office Suite, and the ability to learn other software applications. Knowledge of Epic is helpful.
