Job Title: Full-Time or Part-Time Physical Therapy Assistant – In-Home Mobility Expert

Schedule: Full-Time with weekend rotation
Compensation: $42 - $46 per visit + mileage reimbursement

What You’ll Do:
- Implement personalized treatment plans developed by a licensed Physical Therapist, helping patients regain strength, range of motion, and independence
- Deliver hands-on care including therapeutic exercises, mobility training, gait education, and adaptive strategies to support recovery
- Document all interventions and patient responses in EMR to track clinical progress
- Educate patients and caregivers on home exercise routines and fall prevention strategies, empowering them with confidence
- Collaborate regularly with the supervising PT and interdisciplinary care team to ensure holistic patient care
Qualifications:
- Licensed Physical Therapy Assistant in the state of Minnesota
- Valid driver’s license , reliable vehicle, and current insurance
- Strong communication skills , with the ability to educate and motivate patients
- Home care experience is a plus —but we will train the right individual passionate about making a difference
