About the Job:
The Social Worker is responsible for providing professional social work services to patients of Foundation Pediatric Weight Management to address psychosocial factors affecting medical treatment outcomes.
What You’ll Do:
Practice Social Work in accordance with professional Scope and Standards of Practice, codes of ethics, and relevant state laws governing mental health practice.
Design and implement purposeful intervention plans that assist referred patients and families to gain control over their own circumstances, collaborating with patients and families in the process of bio-psycho-social care planning. Documents social work encounters and plan of care in the medical record.
Provides support, information, advice and practical assistance to referred patients and others as they adjust to the impact of coping with treatment, and assisting them with finding resources for needs: disability, transportation, prescription assistance, financial and Advanced Directives, and Guardianship.
Collaborates with the interdisciplinary team to coordinate care for the psychosocial needs of referred patients.
Who You Are:
MSW Required
Minimum of two years of Professional social work experience.
LICSW in NH and/or MA preferred.
Epic experience, is a plus!
