Quality Improvement Specialist - Long Term Care

This individual is responsible for identifying, implementing, and managing quality improvement initiatives within hospitals, nursing homes, home health, hospice, community coalitions and physician practice settings. The Quality Improvement Specialist works with the senior leadership, direct care, and performance improvement professionals in these settings to ensure the implementation and ongoing internal monitoring of evidence-based interventions in a timely and effective manner. The Quality Improvement Specialist also works with subject matter experts on topics such as infection prevention (including COVID-19), immunizations, drug safety, and other special focuses per CMS to carry out one-to-one quality improvement (QI), small group educational activities, learning and action networks, and resource development. This individual will manage Task 3 work (BFCC, Vaccine, TR QII’s and other TRs) by assigning, reviewing, and ensuring timely responses to all referrals. Partnering with Director to assist with timely, accurate, and complete submission for deliverables.
QUALIFICATIONS:
- Knowledge of the principles and practices of healthcare quality.
- Infection Prevention knowledge and expertise is a plus, as it is a significant portion of the work.
- Clinical experience working in long-term care.
- Superior communication skills (verbal and written) with the ability to communicate quality improvement data and methods to healthcare providers and stakeholders with varying technical and clinical backgrounds.
- Strong time management skills with the ability to multi-task and meet multiple deadlines.
- Ability to work independently with minimal supervision, as well as in a team environment.
- Ability to travel to provider sites and conferences as required.
- Computer proficiency with a developed knowledge of Microsoft applications (Word, Excel, PowerPoint and Access.
- Ability to provide formal and informal presentations to internal staff and external customers.
EDUCATION & EXPERIENCE:
- Licensed, Registered Nurse, required
- Associate’s degree in nursing, public administration, public policy, public health or related field, required. Bachelor’s degree, preferred.
- Two (2) years clinical experience or experience working in a health care setting, required.
- Two (2) years of experience in health care quality improvement, required.
- Certified Professional in Healthcare Quality (CPHQ) or other quality certification, preferred.
The salary range and/or hourly rate listed is a good faith determination of potential base compensation that may be offered to a successful applicant for this position at the time of this job advertisement and may be modified in the future. The annualized salary range for this position is $80,000.00 – $85,000.00. Actual salary range and/or hourly rate depends on a variety of factors including experience, education, credentials, location, and budget.
