In this position you will lead the palliative care team, ensuring excellent patient care services, coordinating educational and outreach services. This position leads quality improvement initiatives, linking team effort with other hospital departments and with outpatient and community services. Works closely with Palliative Care to ensure smooth transition of patients. This position will be responsible for coordinating and completing palliative care consults and proactively identifies opportunities to improve the patient and family experience of care. Responsibilities include but are not limited to working with the Palliative Medicine Physician to ensure appropriate communication between the palliative care service and referring physicians.
This is a full-time, day shift opportunity. This position will require flexibility to work 7.5 hour day shift, 5 days per week, varied start and end times. There is no weekend and Holiday requirement .
Preferred Criteria
- Ability to work independently and collaborate with the interdisciplinary team
- Prior management and leadership experience
- Experience as a Nurse Practitioner (Palliative Care Experience ) and/or Palliative Care RN
Required Criteria
- Master's degree in Advanced Practice
- Current NYS RN license
- Current NYS NP license
- At least 3 - 5 years experience in acute care setting
- Excellent Customer Service
- Certification as Palliative Care NP

Salary Range:
