Physical Therapist (PT) – Home Health

As a Physical Therapist with Healthy Living at Home, you will provide physical therapy assessment, services, direction, and care to patients in your region. You will evaluate the patient’s home environment and functional status, and participate in the development of the total plan of care. Determine priority physical therapy needs and direct physical therapy treatmentProvide physical therapy consultation to patients and their families and caregiversPrepare and submit clinical and progress summaries based on the attainment of goals
Compensation is pay per visit.
Schedule: PRN (weekends)

Requirements

• Licensed to practice as physical therapist within the state
• Doctorate or Master (MS) degree in physical therapy
• Minimum of one (1) year of experience as a physical therapist
• Current CPR certification
• A licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
