Job Summary: Provides direct nursing care to hospice patients, including needs assessment and developing, implementing, and evaluating a plan of nursing care. Provides direct patient care on an ongoing basis in accordance with the Nurse Practice Act and Hospice standards of care. Cultural Behaviors Measurement

Instructs family on homecare techniques and/or patient care techniques used in the inpatient hospice facility.

Informs patient’s physician of any pertinent change in their condition.

Participates in ensuring that nursing services available on an ongoing basis by making regular home visits/inpatient rounds/assessments as needed and ordered by physician and as ordered by department policy. assists in providing 24-hour on-call and/or inpatient staffing service 7 days per week.

Medical Record Audit
Specifications:
1. Education: High School Diploma or GED is required. Associate’s degree in nursing.
2. Experience: Two years nursing experience. Hospice or home health experience and CHPN certification preferred.
3. Certification/License/Registration: Must be licensed as a Registered Nurse in the state of Georgia.
4. Position Titles and Number Supervised: None.
5. Special Training/Skills: Ability to work well within an interdisciplinary team setting as well as independently.
6. Physical Demands: Must be able to use visual near and distance acuity. Must have verbal ability to communicate in person and over the phone or radio. Must be able to assist co-workers in transfers and transport supplies, patients/residents, or equipment using good body mechanics. Must have functional range of motion of the cervical, thoracic and lumbar spines, upper and lower extremities with a grip strength of 60-80 pounds, specific to the job class evaluation. Must be able to forward reach, overhead reach, bend, squat, kneel and change from one position to the other rapidly. Must be able to lift 25-60 lbs. and carry 25-50 pounds specific to job class evaluation. Must be able to tolerate high noise, busy atmospheres and noxious odors. Must be able to work i
