Scheduled Weekly Hours:

Gundersen Health System (becoming Emplify Health) is seeking a casual call, skilled Social Worker for our Hospice team who is committed to excellence, enjoys teamwork, and contributes daily to our mission, vision and values.   

What you will work:  

• This position is casual call with a preference to be able to work at least one shift every week or pay period, primary day shifts.

What you will do:   

• Provide assessment, planning, consulting, education and referral services to Gundersen Health System patients and family members

• Act as consultants and leaders in comprehensive care throughout the system, while supporting organizational strategies and goals

What you will need:  

• Bachelor's degree in Social Work from an institution accredited by the council on Social Work Education

• OR

• Bachelor's degree in psychology, sociology, or other field related to social work and is supervised by an MSW 

• OR

• Bachelor's degree from a school of social work accredited by the Council on Social Work Education, is employed by the hospice before December 2, 2008, and is not required to be supervised by an MSW.WI Social Worker license  

• 1 year of social worker experience in a health care setting

• Social Worker license om both Wisconsin and Minnesota
