Summary

This position supports Mercy's philosophy of patient centered care by providing support to customers, physicians and staff of Mercy Medical Center. The Hospice Nurse Navigator will serve as a patient care nurse, charge nurse and/or shift supervisor as well as leadership on call. The Hospice Nurse Navigator will help guide staff through their Mercy Medical Center journey and help demonstrate leadership skills by being a clinical expert and successful role model of the unit. In addition, may fill the role as the Clinical Supervisor as defined by ACHC.

Job Duties

• Ensures that accurate and clinically relevant information is communicated with patients, their families, and co-workers at hand offs and other times as needed. 
• Takes all measures necessary to ensure that the appropriate and involved parties have communicated necessary information in order to allow for the best possible patient outcome.
• Participates in the evaluation, development, review and/or revision of relevant standards of care and best practice. 
• Demonstrates competent assessment, intervention and evaluation clinical skills, troubleshoots complex problems and complex situations. 
• Prepares patient care assignments and address staff needs. 
• Fosters interdisciplinary, collaborative relationships with internal and external customers. 
• Accountable for ensuring institutional compliance with professional, regulatory and government standard of care, and submission of data as needed. 
• Coordinates or assist with development/ revision of documentation and/or implementation and monitoring of changes in documentation. 
• Completes and provide ongoing coaching and feedback to staff. 
• Provides education regarding patient centered, evidenced based end of life care to employees, volunteers, and the community. 
• Facilitates and nurture relationships with community partners (i.e. facilities) and care providers. 
• Works in collaboration with interdisciplinary team to determine hospice eligibility, certification, re-certification, and to address symptom management.
• Coordinates admissions and the initial assignment of hospice personal to patients and conversely works collaboratively with primary interdisciplinary team to coordinate the discharge of these services when necessary. 
• Engages in assisting and leading quality improvement project, audits, and tracking quality measures. 
• Demonstrates critical thinking in assessing and managing interdisciplinary team and resources to effectively address end of life symptoms, orchestrating a holistic approach to hospice care.
• Demonstrates and sets an example of accountability in time management, flexibility in scheduling and good organizational skills to meet the ever-changing needs of hospice care. 
• Maintains professional boundaries while providing kind and compassionate support and education to employees, patients and families regarding end-of-life care needs. 
• Demonstrates timely and effective collaboration with physician offices, hospice medical director and other members of the hospice interdisciplinary team to manage patients' symptoms and execute the plan of care.
• Works in collaboration with Nurse Manager to ensure staff orientation and competencies are completed timely.
• Follows Mercy's safety guidelines, carries out job-specific safety duties and responsibilities, and promptly reports any unsafe conditions, situations, incidents and injuries.

• Sound background in nursing principles and procedures and demonstrated ability to provide safe and effective basic nursing care.

Professional Experience

• Minimum of two years of hospice/home care experience required.
• At least one year of supervisory experience required.

Education

• Bachelor of Science in Nursing (BSN) preferred.
