Job Description

The BCVI Clinical Nurse Coordinator  reflects the mission, vision, and values of Northwestern Medicine, adheres to the organization’s Code of Ethics and Corporate Compliance Program, and complies with all relevant policies, procedures, guidelines, and all other regulatory and accreditation standards.

The BCVI Clinical Nurse Coordinator is a registered professional nurse who in addition to providing direct patient care, is accountable for the assessment, planning, coordination, implementation, and evaluation of a plan of care for designated patients. The responsibilities and duties include serving as the point of contact for referring providers, processing new patient referrals, and organizing clinical conferences and meetings. This position coordinates screenings, additional testing requirements, consult appointments, and hospital-based procedures. Facilitates communication with multiple specialty physicians including referring physicians regarding patient status for procedures.

The BCVI Clinical Nurse Coordinator will utilize and manage available data to help drive clinical workflows or work plans by organizing, documenting and presenting the information received and reviewing it with the cardiovascular team to determine appropriate patient care pathways.

Under the supervision of the Medical Directors, Director, and the Clinical Program Manager of BCVI the BCVI Clinical Nurse Coordinator will coordinate care for select cardiovascular patients demonstrating the skills below:

  • Establishes and maintains a courteous and professional support with patients, family members, visitors, co-workers, and referring physicians.
  • Excellent interpersonal and communication skills required for patient education.
  • Possesses computer skills to prepare spreadsheets and enter data.

Responsibilities:

  • Responsible for planning, coordinating, directing, and providing appropriate clinical care to patients in the designated care setting.
  • Works collaboratively with the interdisciplinary team to coordinate ongoing care, focused on cost effective ways to maximize wellness and enhance patients’ lives.
  • Develops, coordinates, and provides education for patients, families, caregivers, and staff based on personalized needs along with serving as a resource for ongoing education.
  • Serves as a communication link between the patient, family, caregivers, and interdisciplinary team.
  • Assists in coordinating comprehensive care for patients including any required follow-up visits, referrals, testing, treatment, and/or long-term follow-up.
  • Schedules and coordinates clinical conferences as needed. Prepares and organizes all meeting content, sets agenda and creates PowerPoint presentations to be discussed.
  • Utilizes available sources of data to identify and track patients with cardiovascular conditions to ensure appropriate care plans for patients.
  • Contributes to department, hospital, or other professional committees and/or quality and/or process improvement initiatives.
  • Ensures clinical practice and patient care is consistent with policies and procedures.
  • Other BCVI Clinical Nurse Coordinator duties as assigned.

Qualifications

Required:

  • Education:
    • Bachelor’s degree in nursing
  • License/Certification:
    • Current IL nursing license
    • Current Basic Life Support certification
  • Experience:
    • Minimum of 2-5 years experience in Cardiovascular surgery and/or Cardiology
    • Experience working in hospital and/or clinic setting with direct Physician interaction.

Preferred:

  • Education:
    • Master of Science in Nursing (MSN)
  • License/Certification
    • Specialty certification in the clinical area of focus
