As a Clinical Data Coordinator & OASIS Reviewer, you’ll play a key role in supporting our commitment to high-quality home health care. This is a primarily remote role that offers the flexibility to work from home while making a measurable impact on patient outcomes.

 

OASIS Certification is required

Responsibilities

What You’ll Do:

 

Review OASIS documentation for admission, resumption, recertification, and discharge assessments for Medicare, Medicaid, and HMO cases.

Ensure all documentation aligns with the Plan of Care and medication profile.

Collaborate with field clinicians to validate documentation accuracy and completeness.

Qualifications

• RN with Bachelor’s degree in Nursing  
• OASIS Certification required 
• 3-5 years of home care experience
• Strong knowledge of OASIS and Medicare/Medicaid guidelines
• Strong computer skills  
• Excellent verbal and written communication skills
• Excellent interpersonal skills
