Martin South Hospital’s Progressive Care Unit cares for patients that require a higher level of monitoring, including telemetry. Patients typically have been transferred from med/surg units, the emergency department, the ICU or a post-procedural area. As a Team Center Coordinator, you will provide a high level of customer service to patients, families, visitors and caregivers to promote and support the highest level of patient experience. In this role, you will perform administrative and clerical duties to ensure efficient operational flow while also providing or assisting with basic direct patient care activities under the supervision of a Registered Nurse (RN). A caregiver in this position works nights from 7:00PM – 7:30AM.
A caregiver who excels in this role will:
Conduct an in-person greeting, welcome and orientation to the unit for newly admitted and transferred patients.
Respond to patient call lights and make rounds.
Assist with processing patient admissions, transfers and discharges.
Assist patients and families with wayfinding and direct them to available resources.
Assemble and maintain patient medical records and other documents related to patient care.
Assist with unit clerical functions and in trouble-shooting equipment failures, such as call systems, portable phones, WOWs and other unit devices.
Provide or assist with basic patient care activities, such as taking vital signs and weights, personal care, activities of daily living, patient transfers and transports, pulse oximetry and point of care testing.
Report abnormal findings or changes in physical, mental or emotional condition to an RN.
